The ADHD–Perimenopause Differential Pathway™

Key takeaways

  1. The ADHD–Perimenopause Differential Pathway™ is a clinical reasoning aid I use to sort out whether a midlife woman's changes in focus, memory, and organization point to ADHD, perimenopause, or both.
  2. The Pathway isn't a diagnostic instrument, and I use it only with clinical involvement, alongside a woman's own doctor.
  3. ADHD is lifelong and often masked, so traits a woman managed for years can surface or get louder as hormones shift during perimenopause.
  4. The Pathway sorts the picture into four profiles: ADHD-Predominant, Perimenopause-Predominant, Mixed / Interactive, or Consider Alternative Explanations.
  5. At Jennifer Becker Coaching, I use it in the Profile Session, before any coaching plan is built.

What is the ADHD–Perimenopause Differential Pathway?

It's a clinical reasoning aid. I weigh two things side by side: signs of ADHD that have been there since childhood or early adulthood, and changes that line up with the hormonal timeline of perimenopause. The goal is to understand what's going on before anyone builds a plan.

I'm a licensed clinician (MS, LCMHCS), a certified ADHD specialist, and a certified menopause coach. I built the Pathway because women kept arriving with the same question: is this ADHD, perimenopause, or both? The two can look alike from the outside and feel alike from the inside.

Why sort it out before building a plan?

A plan built for the wrong problem tends to fall apart. Systems built for lifelong ADHD may miss a newer change tied to hormones. Waiting out hormonal changes may miss an ADHD pattern that was being held together by effort for years.

Many of the women I work with say some version of "the workarounds stopped working." That's a real change, and it deserves a clear look. It's also not anyone's fault. Your doctor wasn't dismissing you. The research on perimenopause and mental health is still an incomplete map.

How does the Pathway work?

I look at your history and your current experience together. I ask what has been true for as long as you can remember, what changed and when, and whether other things, such as sleep, stress, mood, or caregiving load, could be carrying more of the weight than it seems.

It works alongside my other tool, the Midlife Executive Function Profile™, which describes how your executive function is working right now. The Pathway helps sort why. The Profile describes what is happening. I don't publish the questions or scoring for either one, because they're meant to be used with a clinician, not on your own.

What are the four profiles?

The Pathway sorts the overall picture into one of four profiles. A profile is a working understanding, not a label or a diagnosis.

ProfileWhat the picture looks likeWhat usually comes next
ADHD-PredominantA long-standing pattern that was manageable and is now harder to manage.A conversation with a qualified provider about formal evaluation, plus executive function systems built for your brain.
Perimenopause-PredominantChanges that are newer and line up with the hormonal timeline, without a clear lifelong pattern.Sharing a clear picture with your doctor, and support for the day-to-day effects.
Mixed / InteractiveA lifelong pattern and hormonal changes, each making the other harder to see.Coordinated care with your providers, and systems that flex as things shift.
Consider Alternative ExplanationsSleep, mood, trauma, stress, invisible labor, or caregiving load may be driving more of what you notice.Looking at those first, with referrals where they make sense.

What is the Pathway not?

It's not a diagnosis, a lab test, or a replacement for your doctor. I can't prescribe medication, and I don't decide whether hormone therapy or ADHD medication is right for you. A formal ADHD evaluation and any hormone decisions belong with a qualified medical or evaluating provider.

ADHD doesn't start in perimenopause, and perimenopause doesn't cause it. What I see is that traits a woman managed for years can surface or get louder as hormones shift.

Who is it for, and who is it not for?

It may fit if

  • You're in midlife and the workarounds that used to work have stopped working.
  • You've wondered if it's ADHD, perimenopause, or both.
  • Your tests came back normal and you still don't feel like yourself.
  • You want a clear picture before you build a plan, or before you talk with your doctor.

It's not the right fit if

  • You're looking for a diagnosis or medication.
  • You want to score yourself at home. I use the Pathway only with clinical involvement.
  • You're in crisis. Please call or text 988 now.

Not sure which side you're on? The free How Am I Actually Functioning? quiz is a low-pressure place to start. The free Perimenopause Symptom Checklist comes with the Midlife Focus Letter, and it's one you can bring to your doctor.

What happens next?

Start with a free 15-minute call. We talk about what you're noticing and whether the Profile Session makes sense. That's where I use the Pathway and the Midlife Executive Function Profile™, and where I build a clearer picture with you before any coaching plan. If you later want support building systems, the four R's of coaching (Relationships, Rest, Resources, Routines) are where we go from there.

Let's discover what's going on so you can feel more like yourself.

Frequently asked questions

Is the Differential Pathway a diagnosis?

No. It's a clinical reasoning aid, not a diagnostic instrument. It helps me organize what you describe so we can decide what to ask your doctor or another qualified provider.

Do I need an ADHD diagnosis before we start?

No. Many women reach this point without a diagnosis, because ADHD is often masked for years. Sorting out the pattern comes first.

Can you tell me whether I need hormone therapy or ADHD medication?

No. I can't prescribe or recommend medication. I can help you organize what you've noticed so you can bring a clearer picture to your doctor.

Can I use the Pathway on my own?

No. I use it only with clinical involvement. The free quiz on this site is a good place to start on your own.
